Qualified High Deductible Health PlanQualified High Deductible Health PlanIn the past few years since the qualified high deductible health plan (HDHP) was signed into law in 2003 as part of the Medicare Modernization Act, it has helped millions of Americans to afford the monthly premiums of health insurance programs. The original purpose of HDHPs was to lower health care costs by pushing plan members to analyze their health care decisionswhile making insurance premiums more affordable to everyone. Qualified HDHPs are plans that meet the requirements for plan members also to utilize a health savings arrangement or health reimbursement account to help stretch health dollars further. Some critics believe that high deductible health care plans actually hurt consumersbecause they never fully pay the annual deductible before the plan year ends, so they get reduced coverage until that time. However, the vast majority of employers offer a three-tiered choice of health care plans, and HDHPs a re generally the preferred plans offered, outside of HMOs and defined contribution options. How Do HDHP Plans Work? Employers can elect the type of HDHP offered to employees. An HDHP can allow for only in-network coverage, similar to an HMO, or allow for out-of-network coverage, similar to a POS or PPO plan. If a plan has in-network benefits only, members cannot go outside of the network once the deductible is met. For a plan that allows both in- and out-of-network benefits, members will usually receive better benefits by staying in-network. All in- and out-of-network benefits offered through the HDHP plan, including prescription drug coverage if offered, must apply towards the deductible. Many, but not all, HDHP plans will actually cover preventive and primary care physician visits for a low copayment, though this is not necessary. HDHP plans are not meant to cover the initial health care costs such as preventive, specialist and laboratory visits. Instead, they are meant to cov er catastrophic events such as chronic illnesses or extended hospital visits. Plan policyholders are expected to pay medical office and facility payments till the deductible is met. Once members reach the out-of-pocket maximum, all medical services are covered at no costs. What is the HDHP Deductible and Out-of-Pocket Maximum? HDHP plan members have higher annual deductible costs for their health care coverage, as the plan name suggests. The deductible is the amount of money a plan member must spend from his/her pocket before coverage kicks in. At least part of this deductible amount is covered by the HSA or HRA. As part of the legislation, there are tiefpunkt deductible limits set each year and adjusted for inflation, for a plan to qualify as an HDHP. The Internal Revenue Service (IRS) determines the annual limits for high deductible health care plans. Deductible Minimum Individual 2016 - $1,300Family 2016 - $2,600 The annual out-of-pocket maximum is the maximum amount of money the member pays before medical services are provided at no cost. The annual out-of-pocket maximum includes deductibles and coinsurance payments. Not included in the out-of-pocket maximum are lifetime maximum benefits, usual, customary and reasonable (UCR) amounts, existing benefit limits and pre-certification requirements. Like the HDHP deductible minimum, the out-of-pocket maximum is adjusted each year for inflation. Out-of-Pocket Maximum Individual 2016 - $6,550Family 2016 - $13,100 There is also an IRS approved catch up contribution for those who are 55 or older of $1,000 per individual. Benefits for Employers Since HDHPs do not offer extensive health coverage, they offer much lower premiums to the consumer. With the higher deductibles, its thought that plan members are less likely to see a physician unless its medically necessary. Its also thought that patients will seek out health services that offer good value for the dollar. Using an HSA or HRA with the HDHP can help low er premium costs, or seek self-pay options with providers who offer discounts. The HDHP plan means employees pay the lions share of the deductible amount, keeping costs down for everyone.

Practice flexing your self-control muscle by breaking the following bad habitsUsing your phone, tablet, or computer in bed.This is a big one that most people dont even realize harms their sleep and productivity. Short-wavelength blue light plays an important role in your mood, energy level, and sleep quality. In the morning, sunlight contains high concentrations of this blue light. When your eyes are exposed to it directly, the blue light halts production of the sleep-inducing hormone melatonin and makes you feel more alert. In the afternoon, the suns rays lose their blue light, which allows your body to produce melatonin and start making you sleepy. By the evening, your brain doesnt expect any blue light exposure and is very sensitive to it.Most of our favorite evening devices- laptops, tablets, and mo bile phones- emit short-wavelength blue light brightly and right in your face. This exposure impairs melatonin production and interferes with your ability to fall asleep as well as with the quality of your sleep once you do nod off. As weve all experienced, a poor nights sleep has disastrous effects. The best thing you can do is to avoid these devices after dinner (television is OK for most people as long as they sit far enough away from the set).Impulsively surfing the Internet.It takes you 15 consecutive minutes of focus before you can fully engage in a task. Once you do, you fall into a euphoric state of increased productivity called flow. Research shows that people in a flow state are five times more productive than they otherwise would be. When you click out of your work because you get an itch to check the news, Facebook, a sports score, or what have you, this pulls you out of flow. This means you have to go through another 15 minutes of continuous focus to reenter the flow st ate. Click in and out of your work enough times, and you can go through an entire day without experiencing flow.Checking your phone during a conversation.Nothing turns people off like a mid-conversation text glaubenszeugnis or even a quick glance at your phone. When you commit to a conversation, focus all your energy on the conversation. You will find that conversations are more enjoyable and effective when you immerse yourself in them.Using multiple notifications.Multiple notifications are a productivity nightmare. Studies have shown that hopping on your phone and e-mail every time they ping for your attention causes your productivity to plummet. Getting notified every time a message drops onto your phone or an e-mail arrives in your inbox might feel productive, but it isnt. Instead of working at the whim of your notifications, pool all your e-mails/texts and check them at designated times (e.g., respond to your e-mails every hour). Each time you find yourself thinking about a coworker or person who makes your blo od boil, practice being grateful for someone else in your life instead. There are plenty of people out there who deserve your attention, and the last thing you want to do is think about the people who dont matter when there are people who do.Multitasking during meetings.You should never give anything half of your attention, especially meetings.
